Description: 

In this webcast, practitioners are introduced to home environment in order to plan for safer transition to home from the hospital setting. Practitioners are guided through a thorough interview/ questionnaire that can be completed with patients and families to obtain needed information on the home environment in order to assist in identifying potential barriers to a safe discharge home. Practitioners will gain understanding of common accessibility concerns of clients when returning home, including focusing on access into/out of the home, in home navigation, and bathroom and kitchen access/use. Practitioners will learn about available products that can facilitate safer, accessible home environments. A sample case study designed to apply the information learned in this course is also provided.

 

Learning Objective:

·       Identify common environmental barriers to access into and out of a home, within the

            home, and bathroom prohibiting safe discharge to home for individuals with physical

            disabilities.

      ·    Discuss how to assess the home environment from the hospital setting.

·       Identify products and equipment designed to remove environmental barriers for access

            and use of the bathroom and allow for a safe return to home for individuals with physical

            disabilities.
