Bringing together experts from AOTA, SNFs, Rehabilitation Services, and more, this course series will provide you with the necessary information to navigate this important shift in practice!

 

Session Description:

 The speaker describes a process of designing an instrumental activity of daily living (IADL) intervention plan and discharge plan based on the patient’s cognitive level, unique needs, and goals. This session will highlight occupational therapy’s unique and crucial role in addressing functional cognition to positively impact IADL performance and to prepare clients to safely and successfully transition from the SNF to the next living environment or care setting. Participants will understand their unique role in facilitating quality outcomes, such as optimizing IADL function, quality of life, and safety; and reducing falls, anxiety, and readmission.

 

Conference Learning Objectives:


After completing all sessions of the conference, the learner will be able to:

·         Describe the effect PDPM will have on the delivery of occupational therapy services in skilled nursing facilities (SNFs)

·         Understand that PDPM promotes a more holistic view of patients and their needs, rather than focusing only what happens in the therapy clinic

·         Describe how practitioners can meet the PDPM outcomes through patient-driven, evidence-informed, occupation-based interventions in the context of reimbursement requirements

·         Define PDPM quality outcomes and their relationship to OT practice and Medicare program requirements, and understand how to identify, measure, and report occupational therapy quality outcomes in this context

·         Identify the tools needed for designing and delivering patient-driven, evidence-informed, occupation-based evaluations and interventions and make connections to outcomes
