This is the 3rd presentation from the 2019 AOTA Specialty Conference: PDGM & Occupational Therapy: Navigating Payment Changes in Home Health.

Medication management is an essential IADL. Mismanaging medications contributes to emergent hospitalizations and deteriorating health. Occupational therapy brings distinct expertise to home health team efforts to support effective medication management. This management involves more than simply knowing one’s medications or dispensing one’s pills. This session offers strategies and techniques to assess and address medication management in the context of daily routines.

Learning Objectives:

1) Discuss the relationship between medication management and health-system level quality and payment measures

2) Explain the difference between verbalizing knowledge, demonstrating skills, and performing activities routinely in context in regards to medication management

3) Describe a process for analyzing medication management activities and identifying appropriate intervention

4) Describe the role of occupational therapy in home health interdisciplinary efforts to support medication management and medication adherence
