Course Description
Discharge planning is a complex process that is one of the primary responsibilities of the acute care OT practitioner. The Centers for Medicare and Medicaid Services (CMS) require that hospitals include patient's anticipated needs, identification of risk factors, and identification of successful patient outcomes as 3 priorities for the discharge planning process.
Determining the best possible disposition requires early and honest discourse between occupational therapy, the therapy team, medical team, case manager, family, and patient. Recommendations by the practitioner will contribute to the patient’s transition to the next level of care by considering contextual barriers and ways to facilitate optimal and safe participation in self-care and other meaningful occupations.
This course will provide the practitioners with an overview of the role of OT as a team member during the discharge planning process as well as to discuss positive outcomes when OT is involved in discussions.
