OT practitioners offer distinct services to youth with disabilities transitioning from secondary-education settings to adult occupations. However, this population is severely under served. Barriers to providing services often require interventions at an organizational level. Learning how to develop an occupational profile and intervention plan for a transition program will assist participants to advance practice in this setting.
Following this presentation, the practitioner will be able to:
· Describe the creation of an occupational profile for a transition program or school.
· Describe an occupational profile–informed intervention plan for a school-based organization.
